Rules; examinations; qualifications of applicants for examination

The board shall from time to time adopt rules for its own government and for the examination of candidates for licenses to practice dentistry. Any rule altering the nature or increasing the severity of the examination or the subjects to be included therein shall not be enforced until six months after its adoption and public promulgation. The examination of applicants shall be confined to written or oral, or both written and oral, examinations upon subjects properly relating to the science of dentistry, the knowledge of which is necessary to the proper and skillful practice of said science. The board may also require from applicants, as part of the examination, demonstration of their skill in operative and prosthetic dentistry. No person shall be examined by the board unless he is at least 18 years of age,
of good moral character, and shall present to the board a certificate from the Commissioner of Education of this State, showing that before entering a dental college he had obtained an academic education consisting of a four
years' course of study in an approved public or private high school or the equivalent thereof, unless he has been graduated in course with a dental degree from a dental school, college or department of a university approved by the
board, or holds a diploma or license conferring full right to practice dentistry in some foreign country and granted by some authority recognized by the board. Any member of the board may inquire of any applicant for examination
concerning his qualifications, and may take testimony of anyone in regard thereto, under oath, which he is hereby empowered to administer.
Notwithstanding any provision of law to the contrary, no person who is a graduate of an unapproved dental school shall be examined by the board unless he has successfully completed at least two years of study of a board approved curriculum at a dental school, college or department of a university approved by the board, with a dental degree having been conferred by the school.

License to practice Dentistry in Florida

Dentistry

An applicant shall be entitled to take the examination required in this section to practice dentistry in Florida if he/she:

•Is 18 years of age or older;
•Is a graduate of a dental school accredited by the Commission on Accreditation of the American Dental Association or its successor agency, if any, or any other nationally recognized accrediting agency; or is a dental student in the final year of a program at such an accredited school who has completed all the coursework necessary to prepare the student to perform the clinical and diagnostic procedures required to pass the examinations. With respect to a dental student in the final year of a program at a dental school, a passing score on the examinations is valid for 180 days after the date the examinations were completed. A dental school student who takes the licensure examinations during the student’s final year of an approved dental school must have graduated before being certified for licensure pursuant to s. 466.011, F.S.;
•Has successfully completed the American Dental Association National Board of Dental Examiners dental examination, Parts I and II, within 10 years of the date of application. If scores are older than ten years, must retake clinical examination (Part II).
If an applicant is a graduate of a dental college or school not accredited by the Commission on Accreditation of American Dental Association or of a dental college or school not approved by the board, he shall not be entitled to take the examination required in this section to practice dentistry until he/she:

•Completes a program of study, as defined by the board by rule, at an accredited American dental school and demonstrates receipt of a D.D.S. or D.M.D. from said school; or
•Completes a 2-year supplemental dental education program at an accredited dental school and receives a dental diploma, degree, or certificate as evidence of program completion. Refer to Rule 64B5-2.0146, F.A.C. for clarification.
Once qualified, an applicant must successfully complete the following to be licensed as a dentist in this state:

•A written examination on the laws an rules of the state regulating the practice of dentistry;
•A practical or clinical examination, which shall be administered and graded by dentists licensed in this state and employed by the department for just such purpose.
*The practical examination shall include:

•Two restorations, and the board by rule shall determine the class of such restorations and whether they shall be performed on mannequins, live patients. or both. At least one restoration shall be on a live patient;
•A demonstration of periodontal skills on a live patient.
•A demonstration of prosthetics and restorative skills in complete and partial dentures and crowns and bridges and the utilization of practical methods of evaluation, specifically including the evaluation by the candidate of completed laboratory products such as, but not limited to, crowns and inlays filled to prepared model teeth.
•A demonstration of restorative skills on a mannequin which requires the candidate to complete procedures performed in preparation for a cast restoration; and
•A demonstration of endodontic skills.

If the applicant fails to pass the clinical examination in three attempts, he/she shall not be eligible for reexamination unless he/she completes additional educational requirements established by the board.

•The board may by rule provide for additional procedures which are to be tested, provided such procedures shall be common to the practice of general dentistry. The board by rule shall determine the passing grade for each procedure and the acceptable variation for examiners.

Licensure to practice Dentistry in Ohio

Dental Licensure for Graduates of an Unaccredited Dental College Located Outside the United States
The Ohio State Dental Board (Board) will consider applications from graduates of unaccredited dental college located outside the United States who meet the following criteria:

Have taken a basic science and laboratory examination consistent with rules adopted under section 4715.11 of the Revised Code and received a passing score as established by the board:
Have had sufficient clinical training in an accredited institution to reasonably assure a level of competency equal to that of graduates of accredited dental colleges, as determined by the board.
Pass Parts I and II of the National Board examination.
Provide proof of having passed either the "Test of English as a Foreign Language" (TOEFL), with a score of 650; or attainment of level 109 in the "English Language Service Test."
Provide the following notarized credentials in English translation:
Diploma or evidence of graduation from a dental school.
Evidence of being licensed or admitted to practice in the country of graduation.
A complete transcript of all academic grades in dental school with courses completed, along with pre-dental school transcripts.
Further information required:
A Criminal Records Check (BCI&I and FBI) completed by the Bureau of Criminal Identification and Investigation, must be submitted directly the the Board. Results shall be received by the board prior to the issuance of a license to practice. For information on obtaining a background check, you may go the their website: http://www.ag.state.oh.us/business/fingerprint/index.asp.
A certification letter from each of the state boards of dental examiners where you hold or have held a dental license.
Letters of recommendation from each of the local dental associations or societies of which you have been a member during the preceding five years.
You must also send a certified attestation, acceptable to the Board, from the faculty of an ADA-accredited dental college, that you have had a laboratory examination, and clinical training in all phases of general dentistry, and that you presently possess clinical skills at least equal to the graduates of that school.

Since the vast majority of American dental schools do not offer a laboratory examination, and the laboratory examinations that are offered may not be acceptable to the Ohio State Dental Board, it is recommended that the applicant enroll in a two-year program in an accredited dental school that includes clinical training, and which is commensurate with the final two years of dental school.
Proof of immunity to or immunization against the hepatitis B virus.
Prior to any application being presented to the Board, the applicant must pass an examination on Board Statutes and Regulations (Chapter 4715. of the Ohio Revised Code. This examination is available on our homepage at www.dental.ohio.gov under Jurisprudence exams.
Fees for foreign-trained dentists are the same as those for other dental license applicants, and are likewise nonrefundable.

There is no provision in the Dental Practice Act for licensure of foreign-trained dental hygienists. Only graduates of accredited dental hygiene schools may apply for an Ohio dental hygiene license.

Limited Resident's License
The Ohio State Dental Board (Board) may grant a Limited Resident's License to a graduate of a dental school, authorized to practice in another state or country or qualified to take the regular licensing examination in this state, and who furnishes the Board satisfactory proof that he has been appointed a dental resident at an accredited dental college in this state or at an accredited program of a hospital in this state, but who has not yet been licensed as a dentist by the Board.

Any person receiving such Limited Resident's License may practice dentistry only in connection with programs operated by the dental college or hospital at which he is appointed as a resident as designated on his Limited Resident's License, and only under the direction of a licensed dentist who is a member of the staff thereof, or a dentist holding a current limited teaching license, and only on bona fide patients of such programs. If residency program is changed, you must submit a new application for a limited resident's license.

Limited Resident's License applications must be reviewed and approved by the Board. The license is valid from July 1st of the year of issue, through the termination of the residency program.

The nonrefundable fee for the Limited Resident's License is $10.00.

Licensure to practice Dentistry in Minnesota

Dentist:
-Graduate from a dental school that is accredited by the American Dental Association's Commission on Accreditation OR graduate from a non-accredited dental school and complete a certificate from an accredited specialty dental program. -Pass the American National Dental Board Examination, parts I and II.
-Pass, within five years prior to application for licensure, the clinical exam administered by:
~Central Regional Dental Testing Service (CRDTS) or,
~Western Regional Examining Board (WREB), [If passing date is prior to 8/1/07 or pass WREB and show evidence of passing Part I and III of the ADLEX], or
~Northeastern Regional Examining Board (NERB), or
~Southern Regional Testing Agency (SRTA), [If passing date is prior to 1/1/2007].
-Pass the Minnesota Jurisprudence Examination covering the statutes and rules of the Minnesota Board of Dentistry within five years prior to application for licensure.

Licensure Requirements to Practice Dentistry in Virginia

Dental licensure:
An applicant for dental licensure shall be a graduate and a holder of a diploma or a certificate from a dental program accredited by the Commission on Dental Accreditation of the American Dental Association, which consists of either a pre-doctoral dental education program or at least a 12-month post-doctoral advanced general dentistry program or a post-doctoral dental education program in any other specialty.

Dental examinations:
1. All applicants shall have successfully completed Part I and Part II of the examinations of the Joint Commission on National Dental Examinations prior to making application to this board.
2. All applicants to practice dentistry shall satisfactorily pass the complete board-approved examinations in dentistry. Applicants who successfully completed the board-approved examinations five or more years prior to the date of receipt of their applications for licensure by this board may be required to retake the examinations or take board-approved continuing education unless they demonstrate that they have maintained clinical, ethical and legal practice for 48 of the past 60 months immediately prior to submission of an application for licensure.
3. If the candidate has failed any section of a board-approved examination three times, the candidate shall complete a minimum of 14 hours of additional clinical training in each section of the examination to be retested in order to be approved by the board to sit for the examination a fourth time.

Licensure by credentials for dentists:
In accordance with § 54.1-2709 of the Code of Virginia, an applicant for licensure by credentials shall:

1. Be of good moral character and not have committed any act which would constitute a violation of § 54.1-2706 of the Code of Virginia;

2. Be a graduate of a dental program, school or college, or dental department of a university or college currently accredited by the Commission on Dental Accreditation of the American Dental Association.

3. Have passed Part I and Part II of the examination given by the Joint Commission on National Dental Examinations;

4. Have successfully completed a clinical examination that involved live patients;

5. Hold a current, unrestricted license to practice dentistry in another jurisdiction in the United States and is certified to be in good standing by each jurisdiction in which he currently holds or has held a license; and

6. Have been in continuous clinical practice for five out of the six years immediately preceding application for licensure pursuant to this section. Active patient care in the dental corps of the United States Armed Forces, volunteer practice in a public health clinic, or practice in an intern or residency program may be accepted by the board to satisfy this requirement. One year of clinical practice shall consist of a minimum of 600 hours of practice in a calendar year as attested by the applicant.

Temporary permit, teacher's license, and full-time faculty license:
A. A temporary permit shall be issued only for the purpose of allowing dental and dental hygiene practice as limited by §§ 54.1-2715 and 54.1-2726 of the Code of Virginia.
B. A temporary permit will not be renewed unless the permittee shows that extraordinary circumstances prevented the permittee from taking the licensure examination during the term of the temporary permit.
C. A full-time faculty license shall be issued to any dentist who meets the entry requirements of § 54.1-2713 of the Code of Virginia, who is certified by the dean of a dental school in the Commonwealth and who is serving full time on the faculty of a dental school or its affiliated clinics intramurally in the Commonwealth.
1. A full-time faculty license shall remain valid only while the license holder is serving full time on the faculty of a dental school in the Commonwealth. When any such license holder ceases to continue serving full time on the faculty of the dental school for which the license was issued, the licensee shall surrender the license, which shall be null and void upon termination of full-time employment. The dean of the dental school shall notify the board within five working days of such termination of full-time employment.
2. A full-time faculty licensee working in a faculty intramural clinic affiliated with a dental school may accept a fee for service.
D. A temporary permit, a teacher's license and a full-time faculty license may be revoked for any grounds for which the license of a regularly licensed dentist or dental hygienist may be revoked and for any act indicating the inability of the permittee or licensee to practice dentistry that is consistent with the protection of the public health and safety as determined by the generally accepted standards of dental practice in Virginia.
E. Applicants for a full-time faculty license or temporary permit shall be required to attest to having read and understand and to remaining current with the laws and the regulations governing the practice of dentistry in Virginia.
